PROGRAM PARTICIPATION

My child has permission to participate in the Walton Volleyball Program and all associated activities. In consideration of being permitted to participate, I agree to assume full responsibility for any risks, injuries, or damages, known or unknown, which might be incurred as a result of my child participating in the program.

In further consideration of my child being permitted to participate in the volleyball program, I knowingly, voluntarily and expressly waive any claim I may have against the Cobb County School District, Walton High School, Walton Volleyball Booster Club, Inc. and its officers, employees and agents for damages, and injury, including death, that may be sustained as a result of participating in the volleyball program.

I, my heirs, or legal representatives forever release, waive, discharge and covenant not to sue the Cobb County School District, Walton High School, Walton Volleyball Booster Club, Inc. and their respective officers, employees and agents for any injury or death caused by my child’s voluntary participation in this volleyball program.I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above. This agreement remains in effect for as long as my child is participating in the Walton Volleyball Program.
